Please note if your agency is already doing business with Burns & Wilcox and you would like to add a new agency location please complete the Burns & Wilcox Enrollment Kit for the new location.
To become a partner agency with Burns & Wilcox, you must first complete the Burns & Wilcox Enrollment Kit, which includes the following:
- Agency Appointment Profile
- Signed Producer Agreement
- Signed W-9 Form
New Partner Agencies are also required to attach the following documents to their Burns & Wilcox Appointment Application:
- Copy of P&C and Surplus Lines (where applicable) Licenses for all states you conduct business in
- Copy of a current E&O Declaration Page or Certificate, including any named insured endorsement
